Senior Care Coordination Specialist

Responsibilities

• Manage care coordination, clinical assessments, and counseling for high-risk patients with mental health, emotional, or substance abuse needs.
• Develop and implement comprehensive, multi-disciplinary individualized service plans in collaboration with medical providers and healthcare teams.
• Conduct individual and group therapy sessions to assist patients with trauma, substance use, and psychosocial challenges.
• Perform medical case management tasks including initial assessment, client monitoring, and periodic re-evaluation of care plans.
• Coordinate and monitor the unit’s buprenorphine program.
• Provide patient advocacy, referrals, and health education to ensure engagement and retention in care.
• Maintain strict adherence to documentation standards, record-keeping guidelines, and grant program requirements.

Requirements

• Master’s Degree in Social Work, Counseling, or a related field.
• Valid New Jersey state license to practice (LSW or LAC).
• Minimum of two (2) years of post-Master's professional experience.
• At least two (2) years of clinical experience providing care to individuals with HIV/AIDS.
• Minimum of one (1) year of experience in managed care or case management.
• Proficiency in Microsoft Office components.
• Strong organizational, communication, and reasoning skills.

Preferred Qualifications

• Bilingual proficiency in Spanish.
• Knowledge of ICD-10 and DSM-V coding.
