About the role

Responsibilities

  • Support the delivery of high-quality community care and provide personal care with dignity and respect
  • Complete initial risk and needs assessments and conduct regular reviews for individual service users
  • Provide practical supervision, instruction, and support to Care Assistants
  • Shadow new care workers to ensure competence and evaluate practice through routine supervision
  • Work with Care Coordinators to plan effective and realistic care worker rotas
  • Participate in multi-disciplinary reviews of service users' needs
  • Complete essential administrative tasks linked to care delivery and service quality

Requirements

  • Experience conducting risk and needs assessments in home or residential settings
  • Experience working in partnership with service users, families, and healthcare professionals
  • Proven ability to contribute to safe, person-centred care planning
  • Strong professional communication skills and the ability to coordinate care with compassion
  • Full UK driving licence and access to your own vehicle
