Senior Care Advocate

Responsibilities

• Build and nurture long-lasting relationships with healthcare providers, including hospitals, skilled nursing facilities, and rehab centers, to generate referrals.
• Meet with professional referral sources and decision-makers in person, targeting 40-50 contacts per week.
• Follow up on leads and prospective client inquiries, conducting initial intake calls and performing assessments.
• Create personalized care plans and assist clients with senior living placement options, including community tours.
• Represent the company at senior-focused networking events, health fairs, and community gatherings.
• Maintain accurate records of all referral sources and sales activities within the company CRM.
• Collaborate with Operations and Care Management teams to ensure smooth transitions for new clients.

Requirements

• Proven ability to build and maintain professional relationships.
• Reliable transportation for field-based travel throughout central Phoenix, Ahwatukee, and Tempe.
• Proficiency in English.
• Strong communication skills with the ability to present services to both professionals and families.

Preferred Qualifications

• 2+ years of experience in outside sales.
• 2+ years of experience in healthcare or senior care industries.
• Experience using CRM software for activity tracking.
