Senior Backend Software Engineer

About the Company

Alpaca Health enables clinicians to become entrepreneurs, starting in autism care. We help clinicians launch and scale their own clinics by providing AI-powered software, payer contracting, and full back-office infrastructure.

Responsibilities

• Build and maintain critical integrations with healthcare vendors, clearinghouses, and state Medicaid systems
• Own backend systems powering billing, authorizations, eligibility, and provider payouts
• Develop software and AI-powered workflows to automate credentialing, compliance, and care coordination
• Build data infrastructure and pipelines for company-wide reporting and operational visibility
• Design and manage platform foundations including APIs, data models, and event-driven architectures
• Implement AI infrastructure and LLM pipelines for clinical documentation

Requirements

• Experience building in operationally complex domains (fintech, healthcare, insurance, or logistics)
• Strong ability to model complex states, transitions, and business invariants
• High ownership mindset with the ability to drive execution in ambiguous environments
• AI-native approach to software development and workflow automation
• Ability to balance rapid feature delivery with long-term system reliability
• Strong product instincts and user-centric design thinking
• Willingness to work 5 days a week in-office in New York City or San Francisco

Preferred Qualifications

• Experience building AI-powered products in production
• Experience with LLM workflows, agents, or evals
• Experience modeling complex financial or operational systems
• Background in healthcare, EHR, insurance, or revenue cycle management
• Experience at an early-stage startup
