Senior Backend Engineer

Responsibilities

• Architect healthcare commerce systems including subscription, billing, and dynamic pricing infrastructure
• Build clinical state machines for medication ordering, fulfillment, and pharmacy integrations
• Own AWS and Kubernetes infrastructure, including CI/CD pipelines and observability
• Partner with Clinical, Operations, and Product teams to shape technical workflows
• Design backend architecture for AI-assisted agentic workflows to automate clinical operations
• Raise engineering standards through code reviews, testing practices, and technical leadership

Requirements

• 5-7+ years of backend engineering experience
• Experience operating high-stakes production systems with complex domain logic
• Strong expertise in data modeling, schema design, and state transitions
• Proficiency with Kubernetes, Terraform, AWS, and CI/CD
• Experience using frontier AI agentic tools in development workflows
• Ability to work in-person 5 days a week in San Francisco

Preferred Qualifications

• Experience with TypeScript, Node.js, Express, PostgreSQL, Redis, Kubernetes, Terraform, and AWS
• Proven track record of setting technical direction across teams (for Staff level candidates)
