RN or LPN Clinical Liaison

Responsibilities

• Collaborate with clinical and hospice teams to promote smooth transitions from facility to home
• Maintain key accounts and build new business through direct referral source servicing
• Educate key targets on the benefits and quality of care provided by VNA Care
• Develop and implement strategies for building relationships with physicians, referral sources, and managed care
• Participate in developing sales and marketing plans for home health, palliative care, and hospice services
• Represent VNA Care at meetings, presentations, educational programs, and conferences
• Monitor the external environment to assess market opportunities and competitor information
• Respond promptly to customer complaints and participate in service recovery efforts
• Meet or exceed referral and admission targets

Requirements

• Associate’s Degree in a clinical field (Bachelor of Science preferred)
• Licensed to practice in Massachusetts as an RN or LPN
• Minimum of 3 years of clinical experience
• At least 1 year of experience in home health care
