RN-Nurse Navigator

Responsibilities

• Utilize the nursing process to assess, diagnose, plan, implement, and evaluate individualized care plans for patients with complex chronic illnesses.
• Provide education and resources to patients, families, and caregivers to facilitate informed decision-making and overcome healthcare barriers.
• Coordinate care across the continuum to ensure timely access to quality health and psychosocial services.
• Collaborate with multidisciplinary care teams to ensure awareness of patient care plans and promote clear communication.
• Advocate for patient wishes using ethical decision-making models and provide linkages to community resources.
• Participate in clinical audits, programmatic evaluations, and the development of certification and accreditation for assigned departments.

Requirements

• Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing.
• Current California Registered Nurse (RN) licensure.
• Basic Life Support (BLS) certification from the American Heart Association (AHA).
• Minimum of three (3) years of RN experience.

Preferred Qualifications

• Experience in chronic disease management, case management, or utilization management.
• Experience in adult acute care or a specific clinical specialty.
• Specialty practice area certification.
