RN-Nurse Navigator

Responsibilities

• Provide individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers.
• Utilize the nursing process to assess patient data, determine diagnoses, and identify expected outcomes.
• Develop and implement comprehensive plans of care to facilitate informed decision-making and timely access to quality care.
• Facilitate patient education regarding health and disease management, including supporting self-management and behavior modification.
• Collaborate with patients, families, and multidisciplinary care teams to ensure awareness of care plans.
• Monitor patient progress through service plans and re-evaluate effectiveness based on changing clinical needs.
• Provide linkages to community resources and services to promote positive health outcomes.
• Integrate research findings and evidence-based practices into daily nursing care.

Requirements

• Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing.
• Current California Registered Nurse (RN) licensure.
• Basic Life Support (BLS) certification from the American Heart Association (AHA).
• Minimum of three (3) years of experience as a Registered Nurse.

Preferred Qualifications

• Specialty certification in a relevant practice area.
• Experience in chronic disease management or case management.
• Experience in utilization management or adult acute care.
