RN Field Clinical Care Coordinator

Responsibilities

• Engage members face-to-face and telephonically to complete comprehensive needs assessments covering medical, behavioral, functional, and socioeconomic needs
• Develop and implement person-centered care plans to manage chronic health conditions, health promotion, and medication management
• Partner with internal care teams, healthcare providers, and community resources to execute care plans
• Provide education and coaching to support member self-management and healthy lifestyle changes
• Support proactive discharge planning and coordinate care transitions following ER visits or hospital admissions
• Advocate for members and families to ensure their needs and choices are supported by the healthcare team

Requirements

• High School Diploma or GED
• Current and unrestricted independent licensure as a Registered Nurse in Massachusetts
• 2+ years of clinical experience
• Intermediate proficiency with MS Office (Word, Excel, and Outlook)
• Access to a designated quiet, secure home workspace
• Reliable transportation and a valid US driver's license for field travel (up to 75%)

Preferred Qualifications

• Bachelor's or Master's Degree in Nursing
• Certified Care Manager (CCM) credential
• 1+ years of community case management experience with complex needs
• Background in Managed Care or team-based care environments
• Experience utilizing Electronic Medical Records (EMR) and virtual care platforms
