About the role
Responsibilities
- Manage a hospice patient caseload across the Twin Falls/Magic Valley service area
- Complete skilled nursing visits in patients' homes and care settings
- Assess symptoms, coordinate medication and care-plan needs, and communicate changes quickly
- Educate and support patients, families, caregivers, and facility partners
- Collaborate with the interdisciplinary hospice team to deliver consistent, patient-centered care
- Document care accurately in CareEfficient or a similar EMR
Requirements
- Current RN license in Idaho, compact RN license, or a clear path to Idaho RN licensure
- At least 1 year of post-graduation RN experience
- Background in hospice, palliative care, home health, oncology, ICU, ER, med-surg, or skilled nursing
- Ability to work independently in the field
- Reliable local travel in and around Twin Falls and the Magic Valley area

- $80,000-$97,000 per year
