RN Case Manager

Responsibilities

• Manage a designated patient caseload, conducting comprehensive assessments and identifying barriers to discharge.
• Coordinate utilization management and status determination by applying clinical criteria to monitor admissions and continued stays.
• Lead discharge planning and care transitions by arranging post-acute services to ensure safe patient movement.
• Collaborate with interdisciplinary teams, including physicians, social workers, and ancillary services, to optimize length of stay.
• Maintain accurate and timely documentation within the EPIC electronic charting system.

Requirements

• Associate Degree in Nursing (ADN) required; BSN required within 5 years of hire.
• Current State of New Mexico Registered Nurse (RN) licensure.
• Clinical nursing experience in a relevant practice area.
• Experience in utilization review or case management is highly desirable.

Preferred Qualifications

• National Case Management certification.
• Experience with EPIC electronic charting.
• Advanced nursing degree (BSN or MSN).
