RN Case Manager

Responsibilities

• Coordinate comprehensive care for a caseload of patients
• Build rapport with patients, families, physicians, and facilities
• Address physical, emotional, and spiritual needs of patients
• Work collaboratively with an interdisciplinary team
• Perform nursing duties in homes, assisted living facilities, or nursing homes

Requirements

• Graduate of an accredited school of nursing (ASN or BSN)
• Current Registered Nurse license in the State of Michigan
• Minimum one year of experience in a clinical care setting
• Reliable transportation, valid driver’s license, and automobile insurance

Preferred Qualifications

• Previous experience in Hospice or Home Health
