RN Case Manager

Responsibilities

• Supervise Referral Coordinator and oversee referral operations
• Analyze data to identify high-risk and high-need patient populations
• Facilitate treatment plan changes in consultation with medical providers
• Assist patients in accessing care resources, including durable medical equipment and medications
• Serve as liaison between Behavioral Health and Clinical departments
• Coordinate and facilitate case management coordination meetings
• Manage Special Diabetes Program for Indians and other specialized care management funding
• Coordinate Narcan education, distribution, and community outreach
• Provide direct patient care as a registered nurse as needed
• Supervise staff by providing guidance, coaching, and performance evaluations

Requirements

• Graduate of an accredited school of nursing
• Minimum of three years of RN experience in a clinical setting
• Basic Life Support (BLS) or CPR Pro Certification
• Ability to obtain Case Manager certification within three years of hire
• Ability to work 100% onsite in Cordova, Alaska

Preferred Qualifications

• Minimum of one year of case management experience
• Minimum of one year of supervisory experience
• Case Manager certification
