RN Care Transitions Manager

Responsibilities

• Coordinate care and services to align with resident goals and acute/post-acute continuum providers
• Facilitate successful transitions from skilled nursing to home environments
• Manage rehabilitation services, including vendor relationships and team competencies
• Perform post-admission screenings and assess resident baseline activation levels
• Develop coaching relationships with residents and stakeholders to empower participation in care plans
• Oversee discharge planning, case management, and care conferences
• Evaluate resident readmissions and recommend quality improvement (QAPI) actions
• Monitor nursing staff delivery of care and implement corrective actions
• Act as Director of Nursing in their absence

Requirements

• Registered Nurse (RN) license in good standing
• Minimum of 2 years of related nursing experience (RN or LPN)
• 1-3 years of experience in a supervisory capacity
• Knowledge of state and federal laws for skilled nursing facility operations
• Proficiency with computer word processing and database software
• Excellent customer service skills
• Ability to read, write, and speak English

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Expertise in PPS, MDS, OBRA, and OSHA
• Administrative experience
