RN Care Navigator

Responsibilities

• Manage a caseload of members focused on kidney health and related co-morbid conditions
• Conduct inbound and outbound calls to provide education on renal replacement therapies, dialysis, and transplant options
• Create and maintain member-centric care plans, including medication reconciliation and social determinants of health screenings
• Serve as a patient advocate and liaison between members, support networks, and interdisciplinary care teams
• Utilize community resources to assist in care planning and resolve barriers to care
• Document all member data and interventions within the Compass care management system
• Comply with HIPAA privacy laws and all company operational and security policies

Requirements

• Active, unrestricted Registered Nurse (RN) license
• Bachelor's degree
• Proficiency in Microsoft Office tools including Excel, OneNote, Outlook, and Word

Preferred Qualifications

• Certified Case Manager (CCM) designation
• Three years of experience in case management
• Clinical experience in a dialysis center or transplant center
• Experience working with Medicare and Medicaid populations
