RN-Care Coordinator

Responsibilities

• Determine the appropriateness of hospital admissions and advocate for medically appropriate, efficient resource utilization.
• Perform comprehensive assessments of patients' clinical, psychosocial, discharge planning, and financial needs.
• Collaborate with physicians, social workers, and the interdisciplinary team to establish and execute complex plans of care.
• Coordinate safe and timely patient transitions and discharges, identifying and removing barriers to efficient throughput.
• Utilize established medical necessity criteria, such as Interqual guidelines, to manage continued stay reviews.
• Act as a resource to staff and physicians regarding levels of care, quality concerns, and regulatory requirements.
• Facilitate communication between patients, families, and the healthcare team to ensure clear understanding of care plans.
• Document all care management assessments, interventions, and discharge planning within the medical record.

Requirements

• Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing.
• Current California Registered Nurse (RN) licensure.
• Two years of RN experience in an acute care or case management setting.

Preferred Qualifications

• Certification in Case Management.
