About the role

Responsibilities

  • Provide client-centered care and case management, including conducting intake interviews and developing goal-oriented support plans
  • Support families in navigating complex social, mental health, and addiction challenges through assessments and advocacy
  • Connect clients and families to essential community resources, including healthcare, financial literacy, and employment services
  • Host family planning meetings, interagency meetings, and case conferences to coordinate comprehensive support
  • Assist immigrants and refugees with community integration and literacy supports
  • Maintain accurate client case notes, databases, and statistical reporting
  • Perform administrative duties such as filing, preparing meeting minutes, and writing letters of support

Requirements

  • Two-year diploma in a related social service or mental health field (e.g., social work, addictions)
  • At least one year of frontline casework experience focusing on mental health, homelessness, or addictions
  • Police Information Check with Vulnerable Sector Search
  • Current Emergency First Aid with CPR
  • Proficiency in Microsoft Word and Excel
  • Strong verbal and written communication skills with the ability to build trusting relationships
