About the role
Responsibilities
- Deliver person-centred and culturally responsive case management support
- Conduct intake, assessment, and registrations while supporting clients with activities of daily living and recovery goals
- Develop holistic and strengths-based individualised case plans
- Deliver practical group-based program and training sessions and facilitate referrals to specialist services
- Maintain accurate client records, reports, and case management documentation
- Participate in team meetings and training activities to contribute to continuous service improvement
- Build positive and sustainable relationships with clients, colleagues, and external stakeholders
Requirements
- Degree in social work or a related discipline
- Strong understanding of mental health and recovery-orientated practices
- Positive person-centred approach with strong communication and interpersonal skills
- Strong organisational and administrative skills
- Resilience and the ability to work effectively in complex environments
- Ability to work across a 24/7 roster, including days, nights, weekends, and public holidays
