Resident Care Coordinator

Responsibilities

• Schedule patient visits in the EHR to optimize RN and APP time
• Manage inbound calls, text messages, and voicemails
• Monitor and manage inbound and outbound faxes
• Maintain updated historical records in the EHR
• Identify and manage health plan insurance for residents
• Support RN and APP by coordinating patient care
• Manage facilitated telehealth visits via tablet
• Conduct direct RCC-led resident visits
• Foster relationships with caregivers, families, and medical providers
• Drive engagement through education on services and consent forms

Requirements

• High School Diploma or GED
• 2+ years of healthcare experience in care coordination or administration
• Medical Assistant, Patient Care Technician, or equivalent certification preferred
• Valid and current driver's license
• Ability to travel between senior living communities and individual homes
• Proficiency with Electronic Medical Records (EMR) and general technology
• Ability to multitask between clinical visits and administrative duties

Preferred Qualifications

• Experience in geriatric care or senior living communities
• 5+ years of healthcare experience if no certification is held
