Resident Care Coordinator - Rehab Lead - Registered Practical Nurse (RPN)

Responsibilities

• Lead the Rehabilitation program including ambulation, mobility, and fall prevention
• Provide clinical leadership as a Registered Practical Nurse in good standing with the CNO
• Support the front-line PSW team by communicating organizational goals and policies
• Ensure Resident-Directed Care principles are applied in daily practices
• Oversee clinical programs and ensure compliance with Long-Term Care legislation and Ministry of Health requirements
• Participate in quality improvement activities and data collection
• Support, orient, and onboard newly hired PSW Team Members
• Manage PSW performance-related matters including coaching and appraisals
• Act as a point of contact for resident and family concerns
• Maintain accurate documentation regarding resident conditions and care plans

Requirements

• Registration with the College of Nurses of Ontario (CNO) in good standing
• Completed Vulnerable Sector Screening (VSS) within the past 6 months
• Updated TB 2 Step within the last year
• Experience in geriatric nursing and Long Term Care
• Knowledge of LTC legislation and MOH compliance
• Proficiency in PointClickCare and Microsoft Office
• Knowledge of HQO principles and quality improvement activities
