Resident Care Coordinator (Lead QMAP)

Responsibilities

• Assist the Health and Wellness Director with QMAP functions including care needs reporting and MAR audits
• Oversee QMAP practice through auditing, coaching, and training to ensure compliance
• Monitor resident safety and communicate condition changes to families, caseworkers, and doctors
• Ensure medication availability, timely ordering, and organized storage areas
• Assist with developing care plans and supervising resident care staff
• Manage medication destruction and oversee medication management processes
• Participate in on-call rotation

Requirements

• Medication certification (QMAP-A/L, Med Tech, Med Aide, or Nurse Delegation)
• At least one year of medication administration experience
• High school diploma or GED equivalent
• Proficiency in English (reading, writing, and speaking)
• Strong organization and multi-tasking abilities
• Ability to handle sensitive issues and maintain confidentiality

Preferred Qualifications

• One year of experience in elder care
• Proficiency with electronic health record management systems
