Renal Care Coordinator

Responsibilities

• Assess patient knowledge of late-stage CKD and treatments to enable informed decision-making regarding health management and RRT transition.
• Provide support, guidance, and care coordination for patients seeking conservative or palliative care.
• Act as a liaison to ensure patients and families receive comprehensive information on dialysis modalities (home, in-center, peritoneal), kidney transplantation, and access types.
• Organize the nephrology practice's late-stage CKD patient population regarding education, modality selection, and permanent access maintenance.
• Participate in the interpretation of clinical data to improve late-stage CKD care processes.

Requirements

• Minimum of 2 years of previous experience in clinical renal patient care.
• Proven experience in renal transplant, dialysis, or CKD patient care.
• Strong organizational and communication skills.

Preferred Qualifications

• Understanding of diabetes and cardiovascular disease processes.
