Relief Case Worker

Responsibilities

• Plan and implement individual, goal-centered programs for adults with developmental disabilities.
• Support Service Users in workplace settings and within the community to promote independence and inclusion.
• Assist Service Users in meeting emotional, psychological, and physical needs (food, clothing, shelter).
• Maintain accurate daily logs, shift reports, and incident reports using AIMS software.
• Administer and accurately document medications as required.
• Organize transportation for Service Users to and from specific destinations.
• Accompany Service Users to professional appointments and provide necessary documentation.
• Work cooperatively with day program teams, support networks, and residential counsellors.

Requirements

• Developmental Social Worker Diploma or equivalent in a Human Services field.
• Minimum of 2 years of experience working with adults with developmental disabilities and vulnerable populations.
• Current First Aid & CPR/AED certification.
• Non-Violent Crisis Intervention Certification (CPI).
• Valid Ontario Class G Driver’s license.
• Medication administration training and experience.
• Ability to provide a Criminal Records Check with vulnerable sector screening.
• Strong oral and written communication skills.

Preferred Qualifications

• Experience in accessing and networking with community resources.
• Lifts and transfers skills.
• Experience in Personal Care Support.
