Rehabilitation Support Worker

Responsibilities

• Provide hands-on support to stroke patients within the community
• Assist with daily living activities
• Implement rehabilitation exercises as directed by physiotherapists
• Provide manual handling support
• Travel around Tower Hamlets to visit patients in their homes
• Maintain accurate patient records and documentation
• Provide emotional support and encouragement to patients and their families

Requirements

• Previous experience as a rehabilitation support worker
• Comfortable with lone working in the community
• Physically capable of traveling around Tower Hamlets
• Ability to provide manual handling in home environments
• Strong communication and interpersonal skills
• Compassionate and patient-centered approach
• Ability to work independently and within a multidisciplinary team
