Rehabilitation Specialist

Responsibilities

• Gather data, document, and present member goals and objectives in accordance with ISP regulations
• Coordinate services with case managers and parent/guardians regarding member goals, issues, and abilities
• Oversee the screening and prioritizing of referrals
• Document member progress and implement behavior management strategies
• Investigate incidents and perform other duties as assigned

Requirements

• 2 years of previous case management experience
• High School diploma with 5 years related experience, Associate’s degree with 3 years related experience, or Bachelor’s degree with 1 year related experience
• Proficiency in Microsoft Word, data entry, email, and internet use
• Valid driver's license and good driving record
• Willingness to transport participants in personal or agency vehicles
• Strong organizational, time management, and communication skills

Preferred Qualifications

• Previous experience working in human services
• Experience using Credible software
