Rehab Support Worker

Responsibilities

• Implement and monitor patient care plans in collaboration with the rehabilitation team
• Assist patients with daily living activities and rehabilitation exercises
• Maintain accurate and detailed records of patient progress
• Work collaboratively with multidisciplinary teams to ensure holistic care
• Contribute to the development of individualized care plans
• Provide emotional support to patients and their families
• Adhere to all relevant clinical guidelines and protocols

Requirements

• HCPC registration as a Rehabilitation Support Worker
• Proven experience in a similar support role, preferably in rehabilitation
• Valid right to work in the UK
• Excellent communication and interpersonal skills
• Ability to work effectively within a multidisciplinary team
• Strong organizational skills and attention to detail
• Ability to handle sensitive and confidential information
• Proactive and empathetic approach to patient care
