Rehab Care Coordinator

Responsibilities

• Support physical, occupational, and speech therapy teams
• Identify and coordinate patient medical equipment needs
• Manage patient care logistics and conduct documentation audits
• Facilitate community programming
• Assist the Rehab Services Director with quality assurance and standards compliance
• Carry out follow-up treatments and facilitate positioning and adaptive equipment
• Organize and maintain equipment and supplies

Requirements

• Graduation from an AOTA approved Certified Occupational Therapy Assistant program
• Successful completion of field work requirements
• Licensed to practice in the state of Montana
• BLS certification
• Ability to communicate effectively with healthcare providers
• Ability to work independently and creatively

Preferred Qualifications

• Minimum of 2 years experience in the rehab field
