About the role
Responsibilities
- Work with a multidisciplinary team to assess new residents
- Lead the PSW team in providing resident care
- Distribute medication and take vital signs
- Monitor glucose readings and support residents during meal times
- Communicate with residents and families regarding care plans
- Update care plans every 6 months or upon changes in condition
- Accurately chart resident care, treatments, and changes in condition
- Support the team and collaborate with the Home and Community Care association
Requirements
- Registration with the College of Nurses of Ontario (CNO) in good standing
- Completed Vulnerable Sector Screening (VSS) within the past 6 months
- Current CPR and First Aid Certification
