Registered Practical Nurse (RPN) - Long Term Care

Responsibilities

• Build genuine connections with residents and create meaningful moments
• Lead the care team through shift reports, supervision, and mentoring of PSWs
• Assess, plan, and deliver care for residents with complex needs
• Administer medications safely and monitor for effectiveness and side effects
• Manage and document pressure injuries using digital photography
• Perform therapies such as subcutaneous hypodermoclysis, IV therapy, and wound care
• Partner with physicians, nurse practitioners, and therapists to adjust care plans
• Complete timely documentation and maintain current care plans using technology
• Support and educate families during challenging moments
• Mentor new colleagues and students to build a supportive care culture
• Participate in quality improvement huddles to shape care delivery

Requirements

• Registration with the College of Nurses of Ontario in good standing
• Vulnerable Sector Screening check completed within the past 6 months
• TB 2 Step updated within the last year
