Registered Practical Nurse - Long Term Care

Responsibilities

• Build genuine connections with residents and create meaningful moments every shift
• Lead care team shift reports, supervise and mentor PSWs, and drive daily priorities
• Assess, plan, and deliver care for residents with complex needs, ensuring comfort, dignity, and safety
• Administer medications safely and accurately while monitoring effectiveness and side effects
• Manage and document pressure injuries using digital photography to track healing
• Perform therapies such as subcutaneous hypodermoclysis, IV therapy, and wound care treatments
• Partner with physicians, nurse practitioners, and therapists to adjust care plans
• Complete timely, accurate documentation using technology to keep care plans current
• Support and educate families during challenging moments
• Mentor new colleagues and students to build a strong, supportive care culture
• Participate in quality improvement huddles to shape care delivery

Requirements

• Registration with the College of Nurses of Ontario in good standing
• Vulnerable Sector Screening check completed within the past 6 months
• TB 2 Step updated within the last year
