Responsibilities

• Provide client-centered primary care in a non-judgmental manner to priority populations
• Perform clinical assessments and health information gathering for clients with complex needs
• Provide nursing services addressing biopsychosocial needs, including mental health and substance use
• Perform intake assessment activities and provide direct access to primary care
• Complete timely and accurate medical records using the Electronic Medical Record (EMR) system
• Support client safety through adherence to infection prevention and control and safe medication practices
• Participate in case conferences, chart audits, and Quality Improvement Plans (QIP)
• Provide clinical supervision and training for nursing students when applicable

Requirements

• Practical Nursing Diploma
• Current registration with the College of Nurses of Ontario (CNO)
• Experience providing care to individuals facing homelessness or substance use challenges
• Basic CPR certification
• Valid Ontario Driver's License and access to a vehicle
• Ability to work a flexible schedule including at least one evening per week
• Valid vulnerable sector police check

Preferred Qualifications

• Membership in the Registered Practical Nurses Association of Ontario
• Phlebotomy certification
• Minimum of two years of experience as a primary care RPN with complex needs
• Experience in a Community Health Centre (CHC) setting
• Proficiency with Electronic Medical Records (EMRs) and Ontario Telemedicine Network (OTN)
