Registered Nurse

Responsibilities

• Serve as a Home Health Case Manager providing care to patients in their homes
• Manage patient care plans within the Westboro, Southboro, Sudbury, Hudson, and surrounding areas
• Coordinate with patients and their families to ensure high-quality care delivery
• Maintain organized documentation and multi-task effectively in a field setting

Requirements

• Valid Massachusetts RN License in good standing
• Minimum of 1 year of med/surg experience
• Willingness to travel within the assigned territory
• Strong organizational skills and the ability to multi-task
• Compassionate approach to working with patients and families

Preferred Qualifications

• 1 year of acute care experience

• Competitive salary range: $86,000 - $129,000 (adjusted for part-time)
