Registered Nurse

Responsibilities

• Conduct home visits to assess patient needs and provide necessary medical care
• Utilize Case Management, Oasis E documentation, and Home Care Home Base EMR system
• Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans
• Ensure accurate and timely documentation of patient progress and treatment plans

Requirements

• Valid MA RN License in good standing
• Minimum of 1 year of med/surg experience
• Willingness to travel within the Leominster, Fitchburg, and Westminster regions
• Strong organization, multitasking, and compassionate communication skills

Preferred Qualifications

• Previous experience in acute care settings

• Competitive salary range: $86,000 - $129,000 (based on full-time schedule)
