Registered Nurse

Responsibilities

• Serve as a Home Health Case Manager providing care to patients within the assigned territory
• Manage patient care plans with compassion and organization
• Multi-task effectively to meet the needs of patients and their families
• Coordinate care within the Plainville, Wrentham, Mansfield, North Attleboro, Foxboro, Franklin, and Easton areas

Requirements

• Valid Massachusetts RN License in good standing
• Minimum of 1 year of med/surg experience
• Willingness to travel within the assigned territory
• Compassionate approach to patient and family interaction

Preferred Qualifications

• Experience in acute care settings

• Competitive salary: $86,000 - $129,000 (full-time, pro-rated for part-time)
