Responsibilities

• Administer multiple assessments and screening tools within the clinical model
• Interpret assessment responses to support prioritization of member needs
• Develop individualized care plans within a multidisciplinary care team
• Lead internal and joint rounds to ensure optimal and coordinated member experiences
• Utilize electronic systems and technology to deliver a coordinated approach to care
• Build strong, trusting, and empathetic relationships with members
• Serve as the lead coordinator for the multidisciplinary team

Requirements

• Active and unrestricted Registered Nurse license in the State of Massachusetts
• Experience in a clinical or medical setting
• Experience in a multi-disciplinary care model
• Strong communication and organization skills
• Ability to multitask and work under pressure
• Strong technical skills for navigating documentation systems
• Valid driver's license with a safe driving record
• Ability to drive during various times and weather conditions
• State minimum automobile insurance coverage
