Registered Nurse RN - Triage

Responsibilities

• Identify, prioritize, and coordinate patient and family needs to provide preventative and supportive care to terminally ill patients.
• Assess the physical, psychological, social, and spiritual needs of the patient/family unit and initiate appropriate interventions.
• Attend to patient and family complaints and concerns, taking a proactive approach to resolving potential issues.
• Determine health status based on hospice criteria and communicate pertinent information to the Clinical Care Manager and interdisciplinary team.
• Coordinate after-hours services including continuous care, transfers, staffing, physician orders, referrals, pharmacy, and durable medical equipment.
• Educate the community regarding hospice care philosophy and concepts through individual consultation.
• Maintain professional nursing standards and ensure cost-effective care delivery.

Requirements

• Current license as a Registered Nurse in the State of Florida.
• Minimum of two (2) years of nursing experience.
• Training in palliative care and symptom management.

Preferred Qualifications

• Experience in hospice, oncology, or home health nursing.
• Minimum of one (1) year of experience in an acute care unit.
• Minimum of one (1) year of experience caring for terminally ill patients.
