Registered Nurse (RN On Call) Home Care and Hospice

Responsibilities

• Provide on-call coverage for home care and hospice programs during after-hours.
• Triage incoming calls and document all telephone contacts, patient observations, and interventions.
• Prioritize calls and perform patient visits as needed to assess and treat patient needs.
• Document interim verbal orders from physicians accurately into the electronic medical record system.
• Provide skilled professional nursing care to patients, families, and caregivers in compliance with agency policies.
• Complete timely clinical documentation of all after-hours calls and visits.
• Communicate with the interdisciplinary team to ensure proper follow-up on patient needs.
• Notify the after-hours administrator on-call when guidance is required.

Requirements

• Graduate of an accredited school of nursing.
• Current New Hampshire nursing license or a compact state license through the Nurse Licensure Compact (NLC).
• 3-4 years of recent medical/surgical or acute care nursing experience.
