Registered Nurse (RN) - Hospice

Responsibilities

• Deliver age-appropriate hospice care to patients in home or facility settings using standard nursing best practices.
• Develop and implement appropriate plans of care (POC) in partnership with patients, caregivers, and healthcare providers.
• Document accurate and ongoing assessments, treatments, and patient responses in accordance with agency guidelines.
• Educate patients, caregivers, and families regarding hospice philosophy, services, and care instructions.
• Coordinate care with the Interdisciplinary Team, physicians, and other health practitioners.
• Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) assigned to patients.
• Participate in agency projects, committees, and QAPI programs as requested.

Requirements

• Graduate of an accredited school of nursing.
• Current New Hampshire nursing license or a compact state license through the Nurse Licensure Compact (NLC).
• 3-4 years of recent medical/surgical or acute care nursing experience.
• Valid driver's license and a reliable vehicle for travel throughout the service area.
