Registered Nurse (RN) - Hospice Community

Responsibilities

• Provide compassionate, high-quality nursing care to patients in their homes and facility settings
• Assess patient needs and develop individualized care plans to support comfort and dignity
• Provide hands-on nursing care, education, and emotional support to patients and families
• Collaborate with physicians and interdisciplinary team members to coordinate care
• Document patient progress and communicate changes in condition
• Supervise and support LPNs and LNAs assigned to patient care
• Travel throughout the service area to provide in-home patient visits

Requirements

• Graduate of an accredited school of nursing
• Current New Hampshire nursing license or a compact state license (NLC)
• 1-2 years of recent medical/surgical or acute care nursing experience
