About the role

Responsibilities

  • Deliver age-appropriate hospice care to patients in home or facility settings using standard nursing best practices.
  • Develop and implement appropriate plans of care in partnership with patients, caregivers, and healthcare providers.
  • Document accurate and ongoing assessments, treatments, and patient responses in accordance with agency guidelines.
  • Coordinate care with the Interdisciplinary Team and communicate effectively with physicians and other health practitioners.
  • Supervise Licensed Practical Nurses and Licensed Nursing Assistants assigned to patients.
  • Educate patients and caregivers regarding hospice philosophy, services, and care instructions.
  • Safely operate a personal vehicle to travel throughout the service area for patient visits.

Requirements

  • Graduate of an accredited school of nursing.
  • Current New Hampshire nursing license or a compact state license through the Nurse Licensure Compact (NLC).
  • 3-4 years of recent medical/surgical or acute care nursing experience.
  • Valid driver's license and a reliable vehicle for community-based travel.
