Registered Nurse (RN) - Home Care

Responsibilities

• Develop and implement appropriate plans of care in partnership with patients, healthcare providers, and caregivers
• Perform thorough assessments, interventions, and evaluations within the scope of practice defined by the NH Board of Nursing
• Document accurate and ongoing assessments, treatments, and patient responses in accordance with agency guidelines
• Engage patients and caregivers in meaningful education regarding their care plans
• Coordinate care with the interdisciplinary team and communicate effectively with ordering providers
• Supervise Licensed Practical Nurses and Licensed Nursing Assistants assigned to patients
• Safely operate a personal vehicle to travel throughout the service area for patient visits

Requirements

• Graduate of an accredited school of nursing
• Current New Hampshire nursing license or a compact state license (NLC)
• One year of recent general medical/surgical or skilled/acute care nursing experience
• Valid driver's license, reliable transportation, and an acceptable driving record
