Registered Nurse (RN) - Home Care

Responsibilities

• Deliver age-appropriate nursing care in the home setting using assessment, planning, intervention, and evaluation.
• Develop and implement plans of care (POC) in partnership with patients, caregivers, and healthcare providers.
• Document accurate assessments, treatments, patient responses, and educational needs.
• Coordinate care with the interdisciplinary team and communicate effectively with ordering providers.
• Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) assigned to patients.
• Engage patients and caregivers in meaningful education regarding their care plans.
• Safely operate a personal vehicle to travel throughout the service area for patient visits.

Requirements

• Graduate of an accredited school of Nursing.
• Current New Hampshire nursing license or a compact state license through the Nurse Licensure Compact (NLC).
• One year of recent general medical/surgical or skilled/acute care nursing experience.
• Valid driver's license, reliable transportation, and an acceptable driving record.
