Registered Nurse (RN) Home Care

Responsibilities

• Deliver individualized one-on-one home health care including assessment, planning, intervention, and evaluation.
• Develop and implement appropriate plans of care in partnership with patients, healthcare providers, and caregivers.
• Document accurate and ongoing assessments, treatments, and patient responses to care.
• Engage patients and caregivers in meaningful education related to their care plans.
• Coordinate patient care with the interdisciplinary team and communicate effectively with ordering providers.
• Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) assigned to patients.
• Safely operate a personal vehicle to travel throughout the service area for patient visits and meetings.

Requirements

• Graduate of an accredited school of Nursing.
• Current New Hampshire nursing license or a compact state license (NLC).
• One year of recent medical/surgical or acute care nursing experience.
• Valid driver's license and reliable transportation with an acceptable driving record.
