Registered Nurse (RN) - Home Care

Responsibilities

• Develop and implement appropriate plans of care in partnership with patients, caregivers, and healthcare providers
• Perform thorough assessments and document accurate nursing interventions, patient responses, and progress toward goals
• Provide meaningful education to patients and caregivers regarding their care plans
• Coordinate care with the interdisciplinary team and communicate effectively with ordering providers
• Supervise Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (CNAs) assigned to patients
• Participate in agency projects, committees, and continuing education programs
• Safely operate a personal vehicle to travel throughout the service area for patient visits and meetings

Requirements

• Graduate of an accredited school of Nursing
• Current New Hampshire nursing license or a compact state license through the Nurse Licensure Compact (NLC)
• At least one year of recent general medical/surgical or skilled/acute care nursing experience
• Valid driver's license and reliable transportation
• Acceptable driving record
