Registered Nurse (RN) - Home Care and Hospice

Responsibilities

• Provide on-call coverage for home care and hospice programs during after-hours.
• Triage incoming calls and document all telephone contacts, observations, and interventions in the electronic medical record system.
• Prioritize calls and perform patient visits as needed to assess and treat patient needs.
• Document interim verbal orders from physicians accurately and communicate them to the interdisciplinary team.
• Process referral information requests and notify the Intake Nursing Staff.
• Provide skilled professional nursing care to patients, families, and caregivers in compliance with agency policies and physician prescriptions.
• Maintain accurate on-call logs and complete clinical documentation for all after-hours visits and calls.
• Communicate clearly with professional staff, physicians, and administrators regarding patient status and expected call-backs.

Requirements

• Graduate of an accredited school of nursing.
• Current New Hampshire nursing license or a compact state license through the Nurse Licensure Compact (NLC).
• 3-4 years of recent medical/surgical or acute care nursing experience.
