Registered Nurse (RN) - Case Manager

Responsibilities

• Complete initial assessments of patients and families to determine home care needs
• Provide complete physical assessments and medical histories
• Initiate and revise plans of care based on changing patient status
• Use health assessment data to determine nursing diagnoses
• Establish goals and incorporate therapeutic, preventive, and rehabilitative nursing actions
• Administer medications and treatments as prescribed by physicians
• Counsel patients and families regarding nursing and related needs
• Identify and implement discharge planning needs
• Prepare clinical notes and update primary physicians
• Communicate with physicians and community health professionals to coordinate care
• Participate in on-call duties as defined by policy
• Instruct, supervise, and evaluate home health aide care
• Review and co-sign LPN visit records and physician orders

Requirements

• Valid Ohio Registered Nurse (RN) license
• Current CPR certification
• Minimum of 1-year clinical nursing experience
• Valid driver's license with a clean driving record
• Reliable automobile and insurance in accordance with Ohio laws

Preferred Qualifications

• Bachelor’s degree in Nursing
• Experience in Home Health, Community Health, or Hospice
