Registered Nurse (RN) - Case Manager

Responsibilities

• Carry a full, high-quality caseload with a target of ~30 productivity points per week
• Perform comprehensive in-home assessments and skilled documentation including OASIS, SOC, recert, ROC, and discharge
• Provide direct, high-acuity patient care with a focus on wound care
• Build and manage plans of care with physicians and supervise LPNs and HHAs
• Drive low avoidable-hospitalization rates and strong patient-experience scores
• Act as a referral-source ambassador to discharge planners, ALFs, and physicians

Requirements

• Active Michigan RN license
• Reliable transportation, valid driver's license, and auto insurance
• Home health or OASIS experience strongly preferred

Benefits

• $85,000-$105,000 approximate yearly pay
