Registered Nurse (RN) Care Manager

Responsibilities

• Coordinate patient care to support safe, seamless, and timely transitions across the healthcare continuum
• Identify, assess, plan, and implement services required to meet individual health needs and social determinants of health
• Prioritize patients in need of care management using a holistic approach inclusive of biopsychosocial and spiritual factors
• Maintain clear, concise, and timely documentation in patient records to reflect care plans and transitions
• Follow standardized practices related to Advance Care Planning, Length of Stay management, and readmission prevention
• Support denial prevention by addressing barriers to care and participating in Interdisciplinary Discharge Rounds

Requirements

• RN License in the state in which you are working or covered by a compact
• BLS Basic Life Support certification from the American Heart Association
• 1 year of experience in a clinical setting

Preferred Qualifications

• 3 years of experience in an acute care clinical setting
• Experience in ambulatory or post-acute care coordination
• Accredited Case Manager (ACM), Certified Case Manager (CCM), or ANCC Nursing Case Management board certification
