Registered Nurse (RN)

Responsibilities

• Conduct skilled nursing visits, assessments, and care planning in client homes
• Provide wound care, medication management, and chronic disease support
• Deliver client education and person-centered care
• Collaborate with caregivers, therapists, and case managers
• Document care accurately in compliance with state and agency standards
• Support continuous quality improvement initiatives

Requirements

• Active NH RN license
• Strong clinical judgment and communication skills
• Compassionate, reliable, and committed to client-centered care
• Ability to work autonomously in a home health setting

Preferred Qualifications

• Previous experience in home health care
