Registered Nurse, Primary Care

Responsibilities

• Provide professional nursing care including patient assessment, treatment, and care planning
• Act as the primary liaison between patients, staff, and physicians
• Obtain and document health history, vital signs, and chief complaints
• Execute prescribed medical interventions and administer medications
• Educate patients and families regarding health status and chronic condition management
• Document all assessment and intervention data using established medical record systems
• Participate in multidisciplinary teams to improve patient care outcomes
• Monitor and track quality assurance data for acute and chronic care management

Requirements

• Active Registered Nurse license in the state of Illinois
• Minimum of one year of professional nursing experience
• Completion of nursing training from an accredited school of nursing
• Active BLS certification from the American Heart Association (AHA)

Preferred Qualifications

• Bachelor's degree in Nursing (BSN)
• Prior experience working in a clinical setting
